- Allotment Memo (issued by CEE / MCC)
- NEET PG Score Sheet
- Data Sheet for State Quota Admission
- SSLC / CBSE / ICSE Mark List or Pass Certificate
- Plus Two (+2) Certificate
- Qualifying Examination Certificate (MBBS Mark List / Pass Certificate)
- Permanent Registration Certificate (TC Medical Council)
- CRRI Internship Completion Certificate
- Transfer Certificate (TC)
- Course & Conduct Certificate
- Eligibility Certificate & Migration Certificate (if applicable)
- Caste Certificate (if applicable)
- Physical Fitness Certificate
- Relieving Order (for Service Candidates)
- Bond for Liquidated Damages & Service Bond
- Aadhaar Card (for identity verification)
- Receipt of Fee Paid to CEE
- Originals of all other uploaded documents/certificates (if any)
- Passport Size Photographs (2 copies)
👨⚕️ Additional Documents for Service Candidates
Service quota candidates must present the following additional documents in original:
- Declaration to the effect that he/she has not undergone a PG Degree course under the Service Quota.
- Service Details issued by the Accountant General, Kerala.
- Details of Service from the Head of the Department in the prescribed format.
